Provisional Cast On Toe
Using a provisional cast-on, cast on 16 sts.
Purl one row. Knit in St st for 6 rows.
Unzip provisional cast on.
(K16, pick up 4 sts along short edge) 2x.
Knit one round even (there are 40 sts).

Toe Shaping
Round 1:
    1: K2, M1R, knit to end of needle
    Needle 2: Knit to last 2 sts, M1L, k2
    Needle 3: Same as needle 1
    Needle 4: Same as needle 2
Round 2: Knit all sts.

Repeat toe shaping rounds until there are 72 sts.

Foot Pattern
Foot detailsRound 1: K1, p1 (p2, k2) 8x, p1, k1, k36
Round 2: Same as round 1
Round 3: K1, p1 (p1, k2, p1) 8x, p1, k1, k36
Round 4: Same as round 3

Repeat foot pattern for 15 repeats or until foot is desired length.

Work heel extension rows 18x (total of 36 rows)

Gusset Decreases
Round 1 (decrease):

Work leg pattern for about 4½-5", ending on round 4.

Cuff

The Henpecked STR would look great in this pattern. I've never knit a toe-up sock. It might be time to try, although I'm pretty chicken in this regard.

This is a wonderful sock - looks great, is very comfy and stays up nicely without binding. Thanks for the pattern!
Please check your gusset instructions, as it seems to me that the decreases are both at the end of their needle when I think one should be at the beginning and one at the end.
